Do I need to arrive early for my first appointment?
Yes. If you are a new patient to the clinic we'd appreciate you arriving approximately 5 minutes prior to your appointment time as we have a short registration form we need you to fill in. If you have a health insurance policy please bring your policy number and any authorisation codes or claim reference numbers with you as we will require these for our records. |
